7/24/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ashiyah. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. Ashiyah appeared to be well groomed and prepared for the session. QMHP-C engaged Ashiyah in a discussion to determine if there were any new behaviors that she needed to discuss with QMHP-C. She reported to QMHP-C that she really did not have anything to mention because she just has been just doing her. 2. Behavior: Ashiyah actively participated in the session as she was cooperative throughout the session as she did not need any redirection throughout the remainder of the session. 1.Intervention: QMHP-C began the session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C encouraged Ashiyah to follow directions and prompted her to display appropriate social interactions such as talking about her feelings instead of arguing. QMHP-C focused on the human need for balance, trust, and meaning in one’s life. QMHP-C assisted Ashiyah with making positive decisions and being proud of achievements. QMHP-C suggested that Ashiyah make a list of preventive behaviors which will aid in achieving balance. 2. Intervention: QMHP-C assisted Ashiyah with making her list while encouraging her to utilize her list. QMHP-C and Ashiyah brainstormed alternative solutions to being impulsive such as thinking of things that make her happy to motivate her to do well. QMHP-C processed and reviewed session with Ashiyah and encouraged her to maintain positive behaviors. QMHP-C presented Ashiyah with several scenarios asking her how she should react to various situations. 1. Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah listened and maintained positive behavior during session. Ashiyah indicated that she felt bad about saying bad things around her mother and verbalized obligation to control her impulsive use of bad things. Ashiyah actively participated in the session by cooperating in discussions and expressing her feelings appropriately. 2. Response: Ashiyah indicated that she fully comprehends what was addressed in today’s session as she stated that she is going to be more respectful to family members and communicate more appropriately. Ashiyah also acknowledged that she understands that poor impulse control can be dangerous because she may put herself in harm’s way by not recognizing how her words may affect others.
